Documentation Forms & Requirements

All requests for accommodations must be supported by information included in the submitted documentation. Documentation must be submitted by a qualified professional by either providing a report of the disability following the guidelines below or by completing a form for that disability. In addition to providing documentation, all students must complete an Affiliation Packet. Affiliations Packets can be requested electronically by using our OSS Quick Response Form, or contacting the main OSS office via e-mail at Completed packets and supporting documentation are to be sent electronically to
