Members of the Horizon Health plans can participate in the NJWELL program to make meaningful changes to wellness habits with program enhancements for eligible members and their covered spouses/partners. NJWELL can help participants achieve holistic well-being including:

• Social connection
• Financial security

Participants of the plan can earn points between November 1, 2022 and October 31, 2023 and be eligible for a Mastercard® Prepaid card worth $250 or more.
