Evaluation must state diagnosis of Autism Spectrum Disorder and be dated within 36 months of this application. The assessment\/evaluation must be completed by a professional with comprehensive training and direct experience in differential diagnosis, such as a psychologist, neurologist, or psychiatrist. The professional completing the evaluation is not a student&#8217;s family member or someone who has a personal or business relationship with the student.<\/li>\n<li>List names and accurate contact information of two references who can attest to college readiness. These must be current mental health therapists, treating psychiatrists, case managers, or special education teachers.
