Office of Specialized Services: Documentation Requirements
All requests for accommodations must be supported by data included in the submitted documentation. Documentation must be current and must include the following:
- Statement of specific diagnosis
- Explanation of the impact of the disability or multiple disabilities on academic performance in a post-secondary setting
- Specific recommendations for accommodations and/or academic adjustments to ensure equal access to college programs and services.
For a physical or sensory disability, or chronic medical condition
Physicians diagnosis and comprehensive report

For a learning disability
Comprehensive psychoeducational evaluation, including results of testing, by a licensed psychologist or certified learning disabilities specialist. IEP alone is not sufficient to establish eligibility. High school seniors are advised to have their school conduct a reevaluation specifically for the purpose of college planning.

For a psychological disability
Diagnosis and comprehensive report from a licensed psychiatrist, psychologist, or psychiatric social worker

For ADD/ADHD
Specific diagnosis of ADD or ADHD based on DSM-IV diagnostic criteria and comprehensive report from a physician
