Nominating statements must be limited to the space provided below. Additional supporting documents, such as biographical sketch, curriculum vitae, or resume (typewritten and single spaced on not more than two 8.5 x 11 inch pages) may be attached. Nominators should take great care to address the specific award guidelines when writing a nomination.

If multiple nominations are received for an individual, the most complete and representative one will be forwarded to the selection committee. Should letters of support be submitted, no more than three will be accepted and each must be limited to one 8.5 x 11 inch page.

The decisions of the Alumni Association Board of Directors or their appointees will be guided by the materials submitted by the nominator within the prescribed guidelines.
